Interventional Cardiology A Randomized Trial Comparing the Impact of a Nonionic (Iomeprol) Versus an Ionic (Ioxaglate) Low Osmolar Contrast Medium on Abrupt Vessel Closure and Ischemic Complications After Coronary Angioplasty

چکیده
METHODS We prospectively evaluated the outcomes of 2,000 patients undergoing percutaneous transluminal coronary angioplasty (PTCA). According to a randomized, double-blind protocol, they received either iomeprol (nonionic; n 5 1,001) or ioxaglate (ionic; n 5 999). Intracoronary thrombus before PTCA was found more often in the iomeprol group (4.2% vs 2.7%, p 5 0.04). No other significant differences between both groups were observed with regard to pre-PTCA clinical and angiographic characteristics.

Randomized Trial of Contrast Media Utilization in High-Risk PTCA
Background—Previous in vitro and in vivo studies have suggested an association between thrombus-related events and type of contrast media. Low osmolar contrast agents appear to improve the safety of diagnostic and coronary artery interventional procedures. However, no data are available on PTCA outcomes with an isosmolar contrast agent. Methods and Results—A multicenter prospective randomized d...

There is an ongoing controversy on the impact of isoosmolar contrast agents on contrast media induced nephropathy (CIN). The COntrast Media and NephroToxicity Following Coronary Revascularization by AngioplaSTy (CONTRAST) randomized 324 patients with impaired renal function undergoing PCI to receive either the iso-osmolar contrast medium iodixanol 320 (Visipaque ) or the low-osmolar contrast me...

Background. Thrombotic complications occurring during coronary angiography and percutaneous transluminal coronary angioplasty (PTCA) are relatively frequent and can be influenced by the type of radiographic contrast media used. Low osmolar contrast media (LOCM), both ionic and nonionic, have been considered to be safer than the older high osmolar contrast media (HOCM), causing less haemodynamic...
